Request Appointment

Our patient portal is the most convenient way to get your medication filled on your time. It is as simple as logging on and making a request, and we will get back to you once it is complete. You can also contact our office during business hours and a message will be forwarded to your physician for approval. Please allow at least 48 hours for refills to be processed. Due to current DEA laws, some medications must be physically picked up and no medications will be refilled after hours or on weekends.

Our office does not require referrals; however, a referral may be a requirement of your insurance plan. If you are unsure if your plan requires a referral, please contact your insurance company. If you are unsure if your plan requires a referral, you may contact our office at 228-230-BONE (2663).

Workers’ compensation, Blue Cross Blue Shield, Medicare, and many others. Please contact the member service department for your insurance to verify that our physicians participate in your particular plan.

Yes, co-pays are collected at the time of service. Your co-pay is generally printed on the front of your insurance card. If you are unsure of the amount of your co-pay, please contact your insurance company.

If you have valid insurance, we will collect any co-pay and/or deductible at the time of service and file your insurance. Otherwise, payment is due at the time of service. If you have questions, or special concerns regarding payment at the time of service, please contact us at 228-230-BONE (2663).

We have partnered with HealthMark Group to ensure accurate and timely completion of your request for all medical and billing records. 

 

You can complete a medical release of information form by visiting any of our office locations. Once we have received your signed consent, your request is sent to HealthMark Group for completion. 

 

You will then be notified by HealthMark Group by email, within 2- 3 days of submitting your request, with instructions on how to retrieve your records.

Yes, we have partnered with HealthMark Group to complete these forms.

 

Please bring these forms to any of our office locations and leave them with our front desk staff. We will submit your form to HealthMark for completion.

 

HealthMark Group will contact you via email with instructions on how to retrieve your forms, once payment is received. 

Fees

  • First Form Fee: $35.00
  • Additional Forms: $35.00
  • Form Updates: $10.00

Initial status of forms is available for 48 hours after submission. Total time to process these forms is approximately seven days.
